1. Applicants must read the Qualifications and Requirements and agree to all items outlined.
2. Applicants must read the Random Drug Testing Policy and agree to its terms and conditions.
3. Applicants must read the Social Media Policy and agree to its terms and conditions.
Please carefully read the Qualifications and Requirements to ensure you are eligible for a Phoenix Scholarship, and that you can and will agree to all the terms and conditions described in that document. After you have thoroughly read the Qualifications and Requirements, Random Drug Testing Policy, the Social Media Policy, the Authorization for Release of Information and the FERPA Consent Form, you are ready to begin the online application below.
PLEASE NOTE: The application requires the submission of an essay. Please complete that essay and save it as a Word doc or a pdf or take a photo of your handwritten essay and save it as a jpg. That document will need to be uploaded to the application before the form can be submitted.
